We have had the privilege of serving the community of Carleton Place and surrounding areas for many years. We are sorry to inform you that our practices at McNeely Medical Clinic are closed effective December 31, 2022. McNeely Medical Clinic has worked very hard over the past years to advocate for rural healthcare, and we share with great disappointment that it has become necessary to close our clinic.
Please take note of the following instructions if you are a previous patient of this clinic or are calling from another agency requiring assistance.
Dr. Asaad Al-Hindawi has relocated his practice to the Westend FamilyCare Clinic. If you are a current patient of Dr. Al-Hindawi you can call the Westend FamilyCare Clinic after December 1st at 613-599-3321 to transfer your records and schedule appointments for the new year. Dr. Al-Hindawi is on vacation until January 9th so if you require urgent care, please go to an emergency department or call 811 for advice. If you require prescription refills please ask your pharmacist if they can provide a temporary refill.
Dr. Christine Tai will be relocating her practice to the Westend FamilyCare Clinic in February 2023. If you are a current patient of Dr. Tai you can call the Westend FamilyCare Clinic after January 1st at 613-599-3321 to transfer your records and schedule appointments for the new year. Dr. Tai is on leave until February 13th so if you require urgent care, please go to an emergency department or call 811 for advice. If you require prescription refills please ask your pharmacist if they can provide a temporary refill.
If you are a previous patient of this clinic looking to have your medical records transferred, please ask your new family physician to fax a consent form to 613-253-4595 to begin the process. The fee for transfer of medical records is $25.00-$40.00, depending on the size of your chart. Note that it may take up to 1 week for a response.

Should you wish to not enrol with your physician at Westend FamilyCare Clinic, but will seek care elsewhere and require a copy of your medical records, McNeely Medical Clinic can either transfer your medical records to your new provider on written request from that provider, or can provide you with an encrypted CD containing lab and diagnostic testing reports, specialist and hospital reports, immunization records and treatment history. The fee for a copy or transfer of medical records is $25.00-$40.00, depending on the size of your chart. Please contact the clinic directly if you wish to obtain a copy of your records be mailed to you directly (by email: mcneelymedicalclinic@gmail.com or voicemail at 613-253-2233).
